with Disabilities
The Rehabilitation
Act of 1973 and its subsequent amendments along with the Americans
with Disabilities Act (ADA) are the basis for Disability Services.
If you need an accommodation
because of a documented disability, you are required to register
with Disability Services Office each semester.
Please do not request accommodations directly from your instructor.
All students receiving accommodations must be qualified through
the Office of Disability Services.

1) Students need to open
and print out our form Disability
Services Form
2) Provide documentation
signed and dated by your doctor or treatment professional with the
following information provided:
1) Diagnosis
2) Treatment or assistance
you are receiving
3) Any medications (if
applicable)
3) Behaviors or limitations
that my impact you during classes
4) Suggestions or ideas
from you doctor on how we can best accommodate you
Accommodations

- Materials in alternative format (Braille printer, JAWS software,
enlargement of materials, Books on tape, Victor Vibe Readers
- Note takers
- Test accommodations (extended time)
- Auxiliary aids
- Tape recording of lectures

not to self-identify the institution is not obligated to
