Admissions Guidelines
Hazard Community and Technical College is an "open admissions"
institution and is available to all area residents.
Admissions opportunities are available to a variety of students
including high school graduates, GED graduates, currently enrolled high
school students, and residents preparing to take the GED. Students should
contact one of the admissions offices listed below to request an application.

Occupational/Technical Programs Selective Admissions
Admission to Hazard Community and Technical College does not guarantee
admission to any occupation/technical program, (i.e., nursing, radiography,
physical therapy assistant, automotive). Admission to these programs is
dependent upon program admissions requirements. Consult the admissions
office or the program coordinator for specific information.
Course Placement Assessment
KCTCS administers a Placement Exam Policy in compliance with the
Kentucky Council on Postsecondary Education (CPE). All degree seeking
students with less than 24 credit hours of college must take a placement
test. The ACT and COMPASS tests are accepted. Placement in English, Reading,
and Math courses are based upon test scores.
Non-Degree Seeking Students
Applicants who desire instruction without wishing to become degree
candidates may be admitted as non-degree students. Non-degree students
are exempt from taking the course placement assessment instrument; however,
all students (including high school students) must meet individual pre-requisites
such as those for entry-level English and mathematics courses.
Non-degree students may become degree students after meeting regular
admissions requirements; however, work taken as a non-degree student will
not, in itself, qualify a person for admission as a degree student. Credit
degree courses earned before a student meets admission requirements will
be counted toward a degree.
Transfer Students
Hazard Community and Technical College welcomes transfer students to its
summer, fall or summer sessions. Transfer students follow the regular
application process, except an official transcript of all transfer college
work must be provided.
Visiting Students
Students who are degree candidates at other colleges may apply
for admission as visiting students. Visiting students complete an admissions
application only and submit a letter of good standing from their home
college.
International Students
Hazard Community and Technical College accepts applications from
international students following the same application procedure as domestic
candidates along with presenting documentation of income, health insurance
and sponsor. Fluency in the English language is essential for admission.
Those students whose native language is not English must submit results
of the Test of English as a Foreign Language (TOEFL). A minimum score
of 500 is required. Information about registration for this test is obtained
by contacting the TOEFL Program at the Educational Testing Service.
U.S. Department of Immigration laws apply to all international
admissions.
Admission with Advanced Placement Test Scores
High school students taking advance placement courses and submitting
required Advanced Placement (AP) scores my received advanced collegiate
level credit. The KCTCS catalog contains the the minimum performance scores
to receive credit.
Financial Aid Services
College Eligibility Guidelines
HCTC offers students a variety of financial assistance programs
including grants, student employment, scholarships, and loans. To be eligible
for financial assistance, a student must:
• Be a U.S. citizen or eligible non-citizen.
• Complete, submit, and have processed the FAFSA (Free Application
for Federal Student Aid).
• Be degree seeking.
• Have a completed admissions file including:
1. Completed KCTCS Admission
Application
2. Verification of high
school or GED graduation
• ACT Test Score (or COMPASS for placement).
• Official academic transcripts from all previously attended
post-secondary institutions.
• NOT in default of any federal student loans.
• NOT in repayment of a financial aid grant.
• Meet the KCTCS Standards of Academic Progress (SAP).
Ability to Benefit Students (Non-GED)
Students who are not high school or GED graduates may establish
eligibility for financial aid under provisions of Ability to Benefit federal
guidelines. To establish eligibility the student must take the COMPASS
placement exam and score at the level indicated in the chart below.

When to Apply
Students must apply every year. It is important to apply early,
before March 1.
Federal Pell Grant
This grant is an award to help you pay for the cost of attending
HCTC. The Pell Grant is not a loan and does not have to be repaid as long
as students maintain satisfactory academic progress. You must apply for
a Pell Grant before you can be considered for any additional federal financial
aid. The amount you are awarded will depend on your Expected Family Contribution,
the cost of your education at HCTC and the number of semester hours in
which you are currently enrolled.
Federal College Work Study
The College Work Study program provides employment opportunities
for those students who qualify. Qualifications for the College Work Study
Program are based on the information submitted in a processed FAFSA. Work
Study employment is based on a 12.5-hour workweek. Students working in
the program are paid a wage of $5.50 per hour. When possible, students
employed in the work-study program are placed in employment related to
their course of study. Employment applications for the College Work Study
program are available in the Financial Aid Offices.
Scholarships
HCTC is pleased to offer many scholarship opportunities to their
students. The processing of scholarships is handled by the HCTC Office
of Resource and Community Development. Application to the HCTC Scholarship
program requires the completion of the FAFSA (financial aid application)
and the HCTC Scholarship Application. For more information call the Office

College Assistance Program (CAP)
CAP is a Kentucky resident's grant program of additional financial
assistance administered by the Kentucky Higher Education Assistance Authority
(KHEAA). Application for the CAP grant program is completed with the submission
and processing of the financial aid application (FAFSA). CAP grant awarding
is completed during the spring and summer each year before a new school
year starts. Priority consideration for CAP grant awards is given to students
submitting the FAFSA by March 15 each year. Community and Technical College
students are allowed a total of 5 semesters of assistance from the CAP
grant program. Students must be enrolled in a minimum of six credit hours
to qualify. Eligibility notices from KHEAA are based on raw data and may
change if initial application data is corrected.
KEES
The Kentucky Educational Excellence Scholarship (KEES) is sponsored
by the Kentucky Higher Education Assistance Authority (KHEAA). Students
can earn scholarships based on their Grade Point Average (GPA) for each
year of high school and bonus awards based on their highest ACT score
achieved before graduating from high school. Students have five years
from the date of high school graduation to use eight semesters of eligibility.
Students enrolled in an approved five-year program will have six years
to use ten semesters of eligibility.

Federal Supplemental Educational Opportunity Grant (SEOG)
This federal grant provides additional assistance to attain an
undergraduate education if you have exceptional financial need. Application
for the SEOG grant program is completed with the submission and processing
of the financial aid application (FAFSA). SEOG awarding is completed during
the spring of each year before the start of school year in August. SEOG
awards are not loans and do not have to be repaid. Students must be enrolled
in a minimum of 6 credit hours to qualify.
Student Loans
Students receiving Pell less than tuition may seek additional help
by applying for a student loan. HCTC participates in the Federal Family
Education Loan (FFEL) Program. Students enrolled in six credit hours or
more may seek low-interest student loans. Students making application
and receiving student loans must understand the loans are to be repaid.
Interest rates for student loans will not exceed 8.25%. Students may apply
for student loans in the financial aid office. Applications are reviewed
for financial aid status and previous academic progress. Students must
have a processed FAFSA (financial aid application), completed loan application,
and attend entrance counseling prior to receiving student loan funds.
Student loan applications and materials may be obtained from the HCTC
financial aid office.
Outside Agency Assistance
Financial assistance in the form of scholarships, grants, and loans
from numerous outside agencies is also available for those qualifying.
An outside agency information sheet is available in the financial aid
office.
Tuition Waiver Programs
HCTC participates in a number of tuition waiver programs offering
tuition assistance to qualifying students. The Tuition Waivers generally
cover the cost of tuition only. The following tuition waivers are available
to qualifying students:
• Senior Citizens Waiver (Kentucky residents 65 years of age or
older)
• Tuition Waiver for Foster and Adopted Children
• KCTCS Employee/Dependent Waiver
• Veterans Waivers

Veterans & Dependent Educational Benefits
VA education benefits may be paid for the pursuit of Associate
in Arts, Associate in Science, Associate in Applied Science degrees, diplomas
and certificates. In all cases, benefits are paid only for the minimum
program requirements. The amount of money students receive for education
benefits differs for each program and is based upon veteran, active-duty,
reserve, national guard, or dependent status. For additional information
contact one of the Financial Aid offices listed above.
Repayment of Financial Aid
Students receiving financial aid (Pell Grants) that do not complete
60 percent of enrolled classes will be required to repay a portion of
their financial aid. Typically, students enrolled in 16-week (semester)
classes must attend and participate in classes through the tenth week.
Students receiving financial aid and receiving all grades of “W’s”
and/or E’s may be required to repay a portion of their financial
aid.
Textbook Charges Deferral
Students with complete and payable financial aid files will be
able to charge books in the Bookstore.
Records and Registration Services
Registration Instructions
Academic advising and registration for new students will
be held before the beginning of each semester. Once a potential
student sends in an application, admissions will follow-up with
a letter telling you what you need for your file, any tests that
need to be taken and how to schedule and when they can register
for classes. Academic advisors will be available to assist
students with course selection in fulfilling their degree.
Continuing Students meet with their academic advisor in November
and April to select courses for the subsequent spring, summer and fall
semesters. Continuous registration is available. Academic advisors during
the summer may be limited. Continuing students are encouraged to register
early during "full service" periods.
Readmit Students with complete admissions files may register continuously
based upon meeting course placement assessment requirements and availability
of academic advisor in area of discipline.
Non-degree and Visiting Students with admissions applications on
file may register continuously based upon course placement assessment
requirements and availability of academic advisor in area of discipline.

Add/Drop Opportunities
Students may add classes during the first five days of a 16 week
semester; the first three days of an 8 week semester; the first two days
of a 6 week semester, and; the first day of a 4 week semester.
Students may drop a course with an automatic grade of "W"
up to mid-term of the semester in which the course(s) is scheduled.
After mid-term through the last day of classes, the instructor
determines the final grade on dropped classes. The instructor may assigned
a "W" or "E". Classes can not be dropped during finals.
To be OFFICIAL the Add/Drop Form must be receipted by the Records
Office.
Final Grades
Final grades are sent via e-mail to students. It is critical
that student activate their student e-mail account at https://students.kctcs.edu.
Students should read and delete their e-mail messages regularly
to ensure that adequate space is available for the receipt of
grades.
Requests for Official Transcripts
Students may request transcripts of their permanent record
by presenting proper identification and signing a request form,
or by submitting a written request through the mail. Telephone
requests are not accepted. Transcripts will not be provided if
there is an outstanding obligation to Hazard Community and Technical
College or to any other college within the Kentucky Community
and Technical College System.
Processing by the next business day cost $3.00 for each
official transcript.. For on demand service, the charge is $5.00
per transcript. Faxed transcripts cost $8.00. Transcripts issued
directly to students or faxed are normally not considered official
by other receiving institutions. Only official transcripts are
mailed. Payment must be received in full before official transcripts
will be released.
In order to properly identify the requestor, written notification
should include current name, name at the time of last attendance,
social security number, date of birth, first and last dates of
enrollment at HCTC and degrees received. The request must also
include the complete address (with ZIP code) of the place where
the transcript should be sent and bear the student's signature.

Application for Graduation
An Application for Credential/Degree must be filed with the Records
Office in order for a student to be considered eligible for graduation.
Students should consult the "Academic Calendar" printed
in the Registration Flyer or on-line for the dates by which you
must have filed your Application for Credential/Degree to be eligible
for graduation.
Commencement
Graduation is conducted in May of each year. Graduates from the
previous summer, fall and spring candidates for degree are invited
to participate. Caps and gowns may be ordered through the Barnes
and Noble ookstore.
Student Government and Campus Organizations
Student participation in the governance of the College
and shaping student life on each campus of Hazard Community and
Technical College is valued and promoted. College committees,
on which students, faculty, and administrators serve, recommend
policy and procedural changes in such areas a safety, student
activities, diversity and housing. Students are members of the
Appeals Board and Judicial Board defined by the Code of Student

Student Government
The Student Government Association serves as a representative
body of the students to reflect the opinions and desires of the
student body and provide a medium through which student opinion
my be presented to the College administration and/or faculty.
The Student Government Association also serves as the official
student body of the College, especially in affairs referred to
the SGA by the administration and/or faculty.
The representative to the HCTC Board of Directors is elected from
SGA presidents from each of the campuses.

Baptist Student Union
This organization of Baptist students open to all in promoting
Christian fellowship in college life. BSU provides activities
for worship, discussion, social life, witness and ministry. Additional

CANSO
Commuter and non-traditional student organization. Promotes involvement
of commuter and non-traditioal students. Club sponsors several
campus activities each semester.
Computer Club
Students work on various projects dealing with computers. They
also assist other studnets and/or various groups who may need
help with a particular comuter project, i.e., Powerpoint presentation.
Phi Beta Lambda (PBL)
As a national postsecondary business organization for students
with an interest in business, membership in Phi Beta Lambda develops
leadership qualities and skills; develops communication skills;
offers state and national conferences and provides opportunities
to participate in professional competitions so students can increase
their levels of knowledge and professionalism along with meaningful
interaction with business and industry. Additional information

Phi Theta Kappa
This is a National Scholastic Honor Fraternity dedicated to encourage
scholarship, leadership and service. Membership is by invitation
to students based on grade point average achieved on college work,
character, and citizenship. The College has two chapters. The
chapter is housed at the Hazard Campus and the chapter is housed

KANS
The Kentucky Association of Nursing Students (KANS) is a pre-professional
organization for those students enrolled in nursing education.
The College has two chapters; one at the Hazard Campus and one
at the Lees College Campus.
Health Occupations Students of America (HOSA)
Health Occupations Students of America is a national organization
for postsecondary students enrolled in health occupations education.
HOSA membership develops effective leadership; develops the ability
to communicate more effectively with individuals and groups; sharpens
the skills needed to seek successful employment in the health
and educational fields, and promotes individual and group achievement.

KEA students are usually interested in teaching as a profession.

Humans Helping Humans (H3)
Student perform community service projects. This student organizations
is housed at the Knott County Branch of Hazard Community and Technical
College.
Ready to Work
The Ready to Work program is a free service to Kentucky Transitional
Assistance Program (KTAP) students enrolling at Hazard Community
and Technical College. This program offers career assistance,
peer mentoring, academic tutoring, lifeskills seminars, career
building seminars, personal improvement seminars and a work study
component for eligible students. Please contact the the Ready

to enroll. The Ready to Work program is a collaborative effort
between KCTCS and Kentucky Cabinet for Families and Children.
AIDS/HIV
What is AIDS?
AIDS is Acquired Immune Deficiency Syndrome - a serious illness
that makes the body unable to fight infections. A person with
AIDS is susceptible to certain infections and cancers. When a
person's body cannot fight infections, they become ill and in
many cases die.
What causes AIDS?
AIDS is caused by the Human Immunodeficiency Virus or HIV.
Transmission
HIV can be spread by:
sexual contact (oral, anal, or vaginal intercourse) with an infected
person when blood, semen or vaginal secretions are exchanged;
sharing a syringe/needle with someone who is infected;
receiving contaminated blood or blood products (very unlikely
now because blood used for transfusions has been tested since
March 1985), and;
an infected mother may pass the virus to her unborn child before
or during birth.
AIDS cannot be spread through casual, non-sexual contact such
as:
sharing food, utensils, or plates;
touching someone who is infected with HIV;
touching or shaking hands;
attending school or working with someone who is infected with
HIV;
donating blood -- (this has never been a risk for contracting
HIV);
using public restrooms, or:
being bitten by mosquitoes or any other insect.
Prevention:
To prevent AIDS:
do not share needles or syringes with anyone;
do not have sexual intercourse except with a permanent partner
who you know is not infected. If you choose to have sex with anyone
else, use latex condoms along with a spermicide containing Nonoxynol-9
every time you have sex, and; educate yourself and others about
HIV infections and AIDS.
Treatment
Early diagnosis of HIV infection is important. Persons with HIV
must get prompt medical treatment. Call your local health department,
or the Kentucky AIDS Hotline at 1-800-654-AIDS(2437) Monday -
Friday, 8 a.m. - 4:30 p.m. for information on testing and treatment.
Voter Registration Information
In accordance with Federal Voter Registration requirements enacted
in H.R. 6 “The Higher Education Amendments of 1998”,
the Kentucky’s Voter Registration Form is available to you

downloading, completing the form, and the address for mailing
it to the appropriate county clerk are available from this web
site.
Student Life Policies
Code of Student Conduct
The Code of Student Conduct, approved by the Kentucky Community
and Technical College System Board of Regents, governs all aspects
of academic and extracurricular activities and defines a number
of offenses and permissible sanctions. The Code of Student Conduct
is administered by the Director of Student Life Services, advised
by the Vice President of Student Services.
With regard to offenses and sanctions, the Code guarantees a student
the right to a due process of fair hearings when charged with
a disciplinary or academic offense.

Policy Statement as a Drug-Free Institution
The College is committed to providing a healthy and safe environment
for its students, faculty and staff. The College system has defined
conduct in relation to the unlawful possession, use, dispensation,
distribution or manufacture of alcohol or illicit drugs. Conduct
that is in violation poses unacceptable risks and disregard for
the health, safety, and welfare of members of the College and
shall result in disciplinary action up and including suspension
or termination.
Campus Smoking Policy
The College is a smoke-free facility. Smoking is not allowed inside
the buildings. Chewing tobacco is prohibited. Designated areas
have been established at each campus for smoking outside buildings.
Smoking in entrance ways is prohibited.
Alcoholic Beverages
Drinking on campus is prohibited by Kentucky law. Rules concerning
intoxicating beverages are clearly stated in the Code of Student

Parking
Students may park in all areas unless prohibited by posted signage.
Parking decals are not required.
Please do not park in spaces designated for Visitors, Campus Vehicles
or disabled parking space unless you secure a College permit.
If you require disabled parking, please bring your state disability
permit registration form to the office listed below.

Vehicles parking in disability spaces without College permit are
subject to fine.
Please do not park in designated fire lanes, service areas, driveways,
or egress to buildings. Vehicles violating these designated areas
are subject to fine and/or towing.
Bookstore
The bookstore is operated on the campus by Barnes and Noble for
the benefit of the students. The bookstore carries required and
recommended textbooks for all courses offered by the College.
The bookstore also has a buy-back period available any time.
Other items and services offered by the bookstore are: course
study aids and guides; College insignia items; nursing supplies;
caps and gowns for graduation and a wide selection of miscellaneous
school supplies such as pens, pencils,, paper, notebooks,etc.

Hours are extended for rush and registration.
Food Services/Vending Machines
In November 2004 a microwave, sandwich vending machine and TV
set were placed in the Jolly Classroom Center student lounge.
The First Federal Center student lounge has also had a microwave
and TV set placed in it for student use.
A Grill Service is located in the First Federal Center on the
Hazard Campus. A Cafeteria is located in Jackson Hall on the Lees
College Campus. We also have a grill located on the Technical
Campus. Hours of service are daily from 8 a.m. to 2 p.m.
unless an upcoming event to be catered has been scheduled.
If this should occur notice will given to students and employees
ahead of time.
Operational hours for grill and cafeteria services may vary during
breaks between semesters and during the summer. Please call ahead
to confirm operational hours during these periods.
Vending machines are located throughout buildings of Hazard Community
and Technical College.
Inclement Weather
During inclement weather, students should listen to their local
radio and television stations for announcements regarding delayed
schedules or school closures.

Tuition and Fees
Due Date for Tuition and Fees
Tuition and fees payment arrangement are required prior to the
start of classes for any semester a student registers.
Accounts may be paid by check, cashier’s check, money order
or credit card in person at the Hazard Campus (Room 205, Jolly
Classroom Building) Lees College Campus (Room 103, Smith Administration
Building).

Always include your name, Peoplesoft® Student ID (or Social
Security Number) on your check, money order or cashier’s
check to ensure proper crediting.
Third party payment, i.e., financial aid, employee assistance,
etc., must be made in advance of payment due dates. Failure to
do so will result in cancellation of registration.

Schedule of Annual Tuition and Fees
The costs of education at Hazard Community and Technical College
are met by tuition, state funds, income from endowments, and current
gifts from alumnae and other friends of the College, and grants
from foundations, corporations, and government agencies. The Kentucky
Community and Technical College System (KCTCS) makes every effort
to limit charges to students, but must reserve the right to set
tuition and fees at the level necessary to support a high quality
of academic and student services programming.

FACTS Payment Plan
HCTC offers a payment plan administered by FACTS Tuition Management.
Payments are automatically deducted from a bank account or credit
card. The cost to participate is $25.00. For full details, pick
up a brochure from any of the Business Office locations.
Refund Policy
Tuition refunds are based upon the length of weeks of courses
enrolled. The chart below reflects the designated refund periods
from the beginning of class. Calendar days are used for calculations
(KCTCS Holidays are excluded).

any future registration until an account is in good standing.
Delinquent accounts are subject to referral to an outside collection
agency, which may charge additional fees.
Academic Probation/Suspension
Academic Probation
Students earning a cumulative grade point average (GPA) below
a 2.0 at the end of a term (semester) will be placed on academic
probation.
Academic Suspension
Students are placed on academic suspension after two consecutive
semesters on academic probation and failure to earn a cumulative
2.0 GPA or a term GPA of at least 2.0.
Students who have been academically suspended may be reinstated
after remaining out of the college for at least a one 16 week
semester. To be reinstated to the College, students must write
an appeal to the Academic Dean before the start of classes. Students
are not allowed to register without being reinstated.
